Which Specialties Have the Most Open Residency Positions for IMGs?

Not all specialties are equally accessible to IMGs. The following specialties have the highest proportion of IMG-friendly programs and the most open positions for international graduates:

  • Internal Medicine — The most IMG-friendly specialty in the US. A large proportion of IM residents are IMGs, and many community programs rely heavily on international graduates to fill their slots.
  • Family Medicine — Particularly strong in rural and underserved areas where IMG physicians are vital to the healthcare system.
  • Psychiatry — Growing demand and chronic shortage of psychiatrists makes this specialty increasingly open to IMGs.
  • Pediatrics — IMG-friendly at many community programs, especially outside major urban centers.
  • Neurology — Solid IMG representation, particularly at academic centers and community hospitals.

Step Scores and Open Residency Positions for IMGs

With USMLE Step 1 now pass/fail, Step 2 CK has become the primary numeric differentiator for IMG applicants. General benchmarks for open positions:

  • Step 2 CK 220+ — Opens community IM, Family Medicine, and Psychiatry programs
  • Step 2 CK 230+ — Competitive at most community programs in IMG-friendly specialties
  • Step 2 CK 240+ — Competitive at academic affiliates and more selective community programs

ECFMG Certification and Open Positions

ECFMG certification is required for IMGs to enter accredited residency programs in the United States. Programs with open positions will verify your ECFMG status before extending an offer. Make sure your certification is current and your USMLE scores are verified before applying.

How IMGs Can Find Open Residency Positions

During SOAP

IMGs who did not match are eligible for SOAP if they applied through the NRMP. SOAP is competitive and moves fast — having your documents ready, a target specialty identified, and program contact information prepared before Day 1 is essential. Our SOAP Survival Kit was designed with this exact preparation in mind.

Outside of SOAP

Many IMGs find positions through direct outreach to programs with open slots throughout the year. Programs with mid-year vacancies — when a current resident departs — often prefer to fill quietly and quickly through direct contact rather than waiting for the next SOAP cycle.

Using a Curated Open Position List

Our weekly-updated member list identifies open positions across all specialties and explicitly flags IMG-friendly programs. Rather than spending hours researching individual program websites, members get a ready-to-use list with contact information, updated every week.

Application Documents for IMG Applicants

As an IMG applying for open residency positions, your application package should include:

  • A clean, well-formatted CV that highlights your US clinical experience prominently
  • A personal statement that addresses your background and commitment to practicing in the US
  • ECFMG certification documentation
  • US clinical letters of recommendation (strongly preferred over international letters)
  • A professional, concise outreach email

Our Application Template Pack includes all of these templates, formatted specifically for the 2026 ERAS cycle.

Frequently Asked Questions

Can IMGs apply for open positions outside of SOAP?

Yes. Open residency positions are available year-round, and IMGs can apply to any program with a vacancy at any time. Direct outreach to program coordinators is the primary mechanism for filling positions outside the official SOAP window.

Do IMGs need US clinical experience to find open positions?

US clinical experience significantly strengthens an IMG application. US letters of recommendation (especially from attendings in your target specialty) are highly valued. While some programs will consider candidates without US experience, having at least one US observership or sub-internship is a strong advantage.

Are there open residency positions specifically for J-1 vs H-1B visa holders?

Most IMG-sponsoring programs support J-1 exchange visitor visas. H-1B sponsorship is less common but does exist, particularly at larger academic centers. When applying to open positions, ask about visa sponsorship early in the process to avoid investing time in programs that cannot support your visa status.
